Description of Fetal Monitoring
Before the invention of the electronic fetal monitor, nurses and doctors periodically monitored the baby's heartbeat themselves by placing a stethoscope or ultrasound Doppler fetal heartbeat detector on the mother's abdomen. However, the instruments could not detect subtle changes or provide continuous surveillance. Fetal monitors, attached to the mother by large belts strapped around her abdomen, record out a cardiotocograph (CTG) graph of the fetal heart rate (FHR), the uterine contractions (TOCO or OP or UP), and the fetal movement£¨FM£©continuously. It can be used to diagnose the functions of fetal nerve-center system and placenta, reflect the state of fetal growth, fetal well-being in womb and the following delivery situation. The monitoring can be classified into the following three: No Simulation Test (NST), Oxytocin Challenge Test (OCT) and Contraction-Simulation Test (CST). Through continuous recordation of the heartbeat, inadequate fetal oxygenation often can be identified and measures taken to avert long-term morbidity or mortality. Developed for high-risk pregnancies, fetal monitoring is applied in the majority of normal pregnancies, too. New attachments, not yet commonly used, may enable them also to monitor the mother's temperature, blood pressure, blood oxygen, heart rate, and cervical dilation.

Time of Fetal Monitoring
Generally, 20 minutes in NST and CST if the result of monitoring is normal; otherwise, stopping monitoring and requiring immediate obstetrical treatment if the result of monitoring is evidently positive; Monitoring continued if the result of monitoring is equivocal until normal result. For high-risk pregnancies, Monitoring longtime in whole delivery procedure, especially in the second delivery procedure.
Key Skill of Fetal Monitoring
¡ï Sound of fetal heartbeat is clear and loud, no noise.
¡ï Location and angel of FHR probe are correct.
¡ï Jelly is enough.
¡ï Strap belts correctly and firm.
¡ï Mother repose on bed sideways, her head overtop body.
